I consent to Cerebral Palsy Alliance and its controlled entities using photographs and/or digital footage of myself and/or my child from the 20/twenty Challenge for promotional, reporting, training and/or commercial purposes, in print, online and broadcast materials and publications.

I understand that photographs and/or digital footage of me and/or my child:

• Could be used in print, digital, social and broadcast materials and publications
• Can be used by Cerebral Palsy Alliance and its controlled entities for a period of up to five years, unless I request these photographs and/or footage no longer be used, via email to janemorgan@cerebralpalsy.org.au or by phone on 1300 888 378.
• May be used by a third party connected to Cerebral Palsy Alliance and its controlled entities for business-related purposes, e.g. in the print, online and social media materials of a corporate supporter or individual supporter of Cerebral Palsy Alliance and its controlled entities.

I also understand:

• I will receive no payment for the use of the photographs and/or digital footage by Cerebral Palsy Alliance and its controlled entities.
• Once a photograph and/or digital footage is published, Cerebral Palsy Alliance and its controlled entities and their staff will have no control over its subsequent use and disclosure.

This permission applies to:

any photographs and/or digital footage photographs and/or digital footage taken during the 20/twenty Challenge
